Our doctors provide guidance and clinical care to help you manage your asthma. They provide information such as tools for self-management and highlight the correct techniques for the use of an inhaler.
Chronic disease management
All our doctors have completed extra training in order to provide care and management for patients suffering from chronic diseases, such as:

We can also assist with referrals to other relevant specialists and health professionals.
We use a dedicated team approach to help you manage and control diabetes. Our doctors and nurses can support you with medical expertise and understanding of how blood glucose, lipids, blood pressure and body weight affect your condition. They also work closely with our dietitians, diabetes educators, endocrinologists (specialists in managing hormone conditions, like diabetes) and our exercise groups. We also run a support group for people living with diabetes.
This involves the Doctor assessing the client, agreeing on management goals with the client, identifying actions to be taken by the client, identifying treatment and ongoing services to be provided and including a review date.
The assessment will identify ways that you can improve your health and wellbeing. It will help by advising how you can maintain your independence at home and in your community. One of our practice nurses can visit you in your home to undertake a comprehensive health check.
The nurse will work closely with you and your doctor to co-ordinate any necessary follow-up consultations to help you manage your health. Speak to your doctor or our practice coordinator today to see if a health assessment is right for you and to make a booking.

Shared maternity care
Shared maternity care is for clients who are pregnant or have recently had a baby. Clients can see a doctor of their choice at Banyule Community Health, who is more familiar with their medical history. It aims to reduce the number of visits and waiting times at hospital outpatient clinics. Some of our doctors are accredited to provide shared antenatal care with the Royal Womens Hospital and Mercy Maternity Hospital.
