PUBLIC NOTICE OF UPCOMING AGED CARE ACCREDITATION

We wish to advise that the Aged Care Quality and Safety Commission will be visiting Banyule Community Health on 10th & 11th November 2022 to assess whether the quality of aged care services (Podiatry, Physiotherapy, Occupational Therapy and Carer’s Support) provided to you meets the Aged Care Quality Standards.

If you are over 65 years of age and in the last 4 weeks you have received one or more of the above stated services at Banyule Community Health, the Commission is interested in hearing from you (or your representative) should you wish to provide feedback on the quality of care and services provided to you.

This process is completely optional, and you can choose to do any of the following:

  • Contact Marita Carmody – Team Leader at Banyule Community Health on 9450 2025 and request a meeting (in person or by telephone) with a quality assessor from the Commission when they visit Banyule Community Health, or
  • Call the Commission on 1800 951 822 and press option 2 to provide feedback over the phone, or
  • Complete the Commission’s consumer experience online survey. You can access the survey via the link for Home Services at the Internet page: https://www.agedcarequality.gov.au/consumer-feedback. You will need the Commission ID: 300551 to access the survey, or
  • Do nothing and choose not to provide feedback on the quality of care provided to you, on this occasion

If you decide to provide feedback, the Commission will respect the privacy of your information and will not share details with Banyule Community Health unless you specifically agree to this, or where we are concerned about your safety, health or wellbeing.
