Cardiovascular disease affects one in six Australians. As part of Heart Week (5–11 May), Holstep Health is encouraging the community to take a simple but important step: book a Heart Health Check with your GP.

To support people living with or at risk of heart conditions, we offer two key programs designed to improve heart health and overall wellbeing.

Well @ Heart: Cardiac Rehabilitation

Well @ Heart is an 8-week cardiac rehabilitation program for people recovering from heart surgery or managing heart disease at the Coburg Leisure Centre.

Each week, you’ll learn about healthy eating, safe ways to exercise, getting back into your normal routine, and handling stress and emotions. We will support you at your own pace. The goal is to help you feel more confident, improve your health, and get back to everyday life.

Our cardiac rehabilitation can help you to:

  • recover after a heart event or surgery
  • improve your heart health and fitness
  • safely build your strength and stamina
  • return to work, hobbies and daily routines
  • feel more confident about managing your health.

Be Well, Live Well: Support for Chronic Conditions

Be Well, Live Well is an 8-week group program for people who have, or are at risk of, chronic cardiac or respiratory disease, diabetes, or those looking to improve their lifestyle.

It combines group and individual education with supervised exercise to support people to:

  • Stay healthy and independent

  • Build long-term healthy habits

  • Recover and feel better after illness or hospital stays
