Holstep Health awarded contract to provide new complex care services!

We’re thrilled to announce that we will be providing a new service – Neighbourhood Care Team Services – to our Hume community. Our team will work with GPs to manage the complex health and social needs of community members.

The Neighbourhood Care Team Services is a new service for Holstep Health and the community. After submitting the tender, we were successful in securing the contract from North Western Melbourne Primary Health Network (NWMPHN) – the largest Primary Health Network in Victoria. This collaboration means that we can improve health outcomes for our community in the Hume region through increased innovation and local solutions.

The two-year contract supported by NWMPHN comes as a response to recent reviews of the primary healthcare system that highlighted the benefits of moving towards preventative models.

The service will be a two-person team of a clinical care coordinator and social worker to help deliver care to patients with complex needs. They will meet weekly with clinicians and patients at four Hume GP clinics, and will assist staff in delivering expert care, supporting patients to manage their own needs, and help to coordinate other support available.

Holstep Health’s Manager of Prevention and Complex Care, Gillon Duguid, said “we’re excited to expand specialist care and partner with GPs to bring more services to even more people in our community. It’s great to partner with NWMPHN in such an innovative service.”

You can expect to see the Neighbourhood Care Team at selected GP clinics over the coming months. We’re excited to bring these services to drive local responses to local community issues.
